IMPORTANT: Consular Birth Registration must be done before applying for a child’s passport. The application will be denied if the BIRTH REGISTRATION has not been made prior to the passport application.
How to request for the passport for children under 18 years ?
By MAIL
At the Brazilian Consulate in Chicago, you should do the following:
1) Visit the E-CONSULAR platform to attach the required forms and the electronic copy of your documents (see list below). Link for the E-CONSULAR platform: https://ec-chicago.itamaraty.gov.br
2) Wait for a notification email from the Consulate with the confirmation and verification of your documents
3) After receiving this notification, prepare an envelope and include the required forms, all the original documents (the same documents you attached at the e-consular platform), the Money Order and a sealed envelope to receive your passport and all your original documents back. Send the package to the Brazilian Consulate in Chicago via the United States Postal Service (USPS).
IMPORTANT: READ THE “FREQUENTLY ASKED QUESTIONS ” below to learn how to collect signatures on the forms.
OBS: If you are requested to be present at the Consulate (if you have an appointment) bring all the original documents with you for verification at the time of your appointment.
IN PERSON
PASSPORT APPLICATION FOR BRAZILIAN MINORS IN PERSON
Due to the restrictions imposed by the pandemic, the consular services have been provided only by mail. For emergency cases, please send an email to passaporte.chicago@itamaraty.gov.br to request an appointment.
If it is necessary to schedule the visit at the Consulate, the procedure for requesting the passport will be very similar to the one mentioned above:
1) Gather the necessary documentation (see here on the link all the documents you will need

2) Access the e-consular platform, to upload the form and documentation and submit them for analysis by the Consulate.
3) After having the documents checked, gather all the ORIGINAL documents (which were attached to the e-consular) and the forms, and BRING them with you on the day of your appointment.
4) Also bring the Money Order to make the payment and a
5) Self-addressed envelope to receive the passport back at your home (if you cannot pick up the passport in person). See important details about the return envelope below.

FREQUENTLY ASKED QUESTIONS & DOCUMENTS NEEDED
FORMS
RER – “Recibo de entrega de requerimento” Form
To start the process of your passport application fill out the Recibo de entrega de requerimento Form (RER) online and print the “Application Receipt” form at the consulate WEBSITE . *If you cannot print the form, write down the RER protocol number.
RER ONLINE FORM : Completing this form is MANDATORY and must be completed and previously signed by the Brazilian parent.
Form “Authorization for passport issuance for minors
The “Authorization for passport issuance for minors” form is MANDATORY, and MUST be completed by both parents. By signing this form, the applicant’s parent or guardian authorizes the Consulate to issue the passport to the child. The signatures in this form will be collected at the Consulate in front of a consular officer. If you are planning to send the application by mail, or if one of the parents (Brazilians or foreign) cannot be present at the Consulate, you need to do the following:
1) The signature(s) need to be verified by a Notary Public AND
2) the Notary Public’s signature needs to be certified by the County Clerk office of your jurisdiction. This process is called Certificate of Authority. It verifies if the Notary has a valid license and if it is registered in your jurisdiction. If you are far away from a County Clerk office, you can go, or send your documents by mail, to the Secretary of State be certified (Apostille).

*The individual who has custody of the minor must present the original court decision.
Travel Authorization for Children
The application for the Travel authorization for minors with only one parent is optional, but it can be requested in the same form “Authorization for passport issuance for minors.”

Birth Certificate of the Child
- For those born in Brazil, must present the Birth Certificate issued by a Brazilian registry office.
- For foreign-born children, parents must present the Consular Birth Registration.
Pictures
You will need One (1) recent photo (size 2×2 inches, taken within the last 6 months), US passport size, front, on white or light background, made professionally. Glue photo into printed form. You will also need a digital copy to attach to the RER form online.
Valid Brazilian Photo ID
A photo ID of the guardians (in the absence of an original document, only certified copies and / or copies certified by a Brazilian notary or by a US Notary Public will be accepted.
*If one of the parents is a foreigner, he/she must present the passport.
*Death certificate of parent (s) if deceased (s).
Previous passport
Previous Passport (if any) expired or about to expire (If unable to present previous passport, police report or “Lost, stolen or destroyed travel document” form, available on the Consulate General’s website at Chicago (chicago.itamaraty.gov.br).
Payment
The Money Order should be issued by the United States Postal Service (USPS) to the Consulate-General of Brazil in Chicago.
IMPORTANT: Money Order must be mailed with the documents. * Do not send money orders separately from documents or documents without Money Order, as they will be returned, and the passport will not be processed.
Personal checks, credit cards or other forms of payment are not accepted.
CONSULAR FEE
$ 40.00, for minors between 0 and 4 years of age
$ 80.00 for minors between 4 and 18 years of age.

You might include Self-addressed envelope: Pre-sealed, with “tracking Number,” via Express Mail.

The turnaround time for applications made in person is five (5) business days; for applications sent by mail, 10 business days.
Passport Validity
Sending Your Documents
For the passport applications sent by mail, you must send to the Consulate all original documentation (the originals of the documents attached to the e-consular platform and the forms). These original documents will be returned by mail to your passport).
For the passport applications requested in person, at the consulate, you must bring with you all the ORIGINAL documents (the ones attached to the e-consular platform, plus the forms) to be verified by the consular officer on the day of your visit.
NOTE: if the parents’ ID contains a name other than that registered on the minor’s birth certificate, a document must be presented that proves the name change (Brazilian marriage certificate or divorce certificate).
* The individual who has custody of the minor must present the original judicial sentence.
The Consulate General may also request OTHER DOCUMENTS if deemed necessary.

IMPORTANT:
• Children 12 and under must be present at the Consulate to collect the fingerprints and signatures.
• For children under 18, parents/ guardians’ original document must be submitted.
• In case of death of one of the parents, the death certificate (original) must be submitted.
REMEMBER: For foreign-born children, parents must present the Consular Birth Registration. That means that parents must register the childbirth at the Consulate before applying for a child’s passport. The application will be denied if the registration has not been made prior to the passport application.
